Upon successful completion and submission of this Alexa Claim Form, eligible Act! subscribers that upgraded to an active Act! Premium subscription or Act! Premium Cloud subscription during the “Move to Subscription” promotion (August 1st – October 31st) will be sent one (1) Amazon Alexa Echo Dot (2nd Generation). You must fill out and submit this Alexa Claim Form in its entirety, including the claim number you received via email, prior to the November 30st deadline to receive your Echo Dot. Offer only valid in the U.S. Eligible Act! subscribers that successfully submit their Alexa Claim Form prior to November 30th will receive one Alexa Echo Dot (2nd Generation) before December 31st.
